This position is located in the Pharmac! y Service.

The Inpatient Acute Care and Surgery Pharmacy Specialist is involved in the independent, professional and clinical practice of pharmacy in accordance with the guidance set forth in Pharmacy Service policies and procedures, VA regulations and State and Federal laws governing the practice of pharmacy. The incumbent is responsible for conducting comprehensive pharmaceutical activities and services in an Acute Care setting with Pre/Post Surgical care requirements included.

Duties include (but are not limited to):
Assures the safe, appropriate, cost effective use of drugs in acute care medicine and surgery patients throughout the inpatient stay including consultative pre and/or post procedure counseling interviews, medication reconciliation through the application of specialized knowledge, the use of judgment in the collection and interpretation of data specific to each patient, and in tactful interactions with other professionals
Prov! ides accurate, up-to-date, relevant drug information and drug literatur! e analysis to health care professionals, and acts as a therapeutic consultant on matters of a high degree of complexity.
Shares responsibility of Medication Use Evaluation with appropriate health care professionals. Actively participates in medical center and pharmacy service's quality assurance programs
Provides educational programs and/or training for VA's medical, pharmacy, and nursing staff including pharmacy residents and students
Participates in clinical drug research and/or provides oversight of pharmacy residents' research projects. Publishes in peer reviewed journals and engages in other scholarly and professional activities on an ongoing basis
Maintains work flow patterns that assure timely delivery of centralized and decentralized pharmacy services, ensures accurate dispensing and record keeping documentation, and oversees technical and nonprofessional staff in the inpatient acute care pharmacy setting
Effectively organizes workload, solves pr! oblems and achieves established goals
Participates in quality improvement activities in all areas of the inpatient acute care pharmacy section

Education

(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certificatio! n, which includes passing the Foreign Pharmacy Graduate Equivalency Exa! mination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).

Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico, or the District of Columbia
English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d)

PREFERRED EXPERIENCE

Residency Training or equivalent experience in acute care clinical practice setting

Board Certification

Basic Life Support (BLS)

Experience precepting pharmacy practice resident and students preferred

GRADE DETERMINATIONS:
GS-11 Pharmacist

Experience, Education, and Licensure. None beyond the basic requirements.

Assignment. Pharmacists ! at this grade level serve in a developmental capacity.

GS-12 Clinical Pharmacist (Full Performance Level)

Experience or Education. In addition to the basic requirements, candidates must meet one of the following:

1. 1 year of experience equivalent to the next lower grade level, or

2. Completion of an ACPE-accredited Pharm.D. program.

Assignment. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record! to identify and solve medication-related problems; contacting provider! s as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution.
