Conifer Health Solutions is currently hiring for an HIM Lead position for Full Time Shifts.
General Summary
Job emphasis is placed on those duties associated with all aspects of clerical duties in the Health Records Information Department: Assembly and analysis of all patient record types, confidentiality ! and release of patient information, retrieval and re-filing of! all patient records, birth certificate processing, transcribed report processing and physician record completion assistance.
Principal Duties and Responsibilities
ESSENTIAL FUNCTIONS - ALL AREAS : Greets and assists patients, visitors and ancillary personnel entering the department; Maintains public relations with all customers entering the department; Answers requests for medical records; Files records in permanent files/designated areas; Obtains necessary patient medical record reports from other hospitals, physician offices and ancillary departments when necessary to complete the medical record or assist in patient care; Answers incoming calls in a prompt and professional manner, routing all calls to their appropriate destination; Enhances the department services through the efficient and effective exchange of ideas and information with others; Communicates in a professional and tactful manner, maintaining confidentiality at all times; Conveys c! ustomer dissatisfaction to supervisor immediately; Assists patients in completing the release of information authorization; Notifies supervisor of supplies needed and equipment repairs needed; Adjusts, files records and shift files appropriately to allow for the filing of records; Changes, as appropriate, the medical record number on patient's records; notifies appropriate personnel of any change; integrates health records with existing files; Performs filing as required; Keeps the department visibly neat; Cross trains in all clerical areas to assist in coverage as needed; Performs all assigned work as required; Retrieves records for the Emergency Department, nursing units and other patient care areas including photocopies of pertinent parts of incomplete records or those on microfilm; Verifies admit/discharge dates for physician offices/insurance companies; Retrieves medical records for quality assurance studies, research, regulatory reimbursement agencies, medical staff c! ommittees, peer review activities, ancillary departments, and physician! offices; This person must be able to calmly and effectively train physicians on the electronic system and must answer questions.
ESSENTIAL FUNCATIONS FOR FILE AREA : Notifies requesting parties of the availability of medical records retrieved for review; Responsible for the proper maintenance of the permanent medical record files by replacing or repairing worn or torn folders; Monitors requestor shelf; Picks up and sorts all incoming department mail and routes to appropriate staff/areas; Verifies identity of individuals receiving information to be released on a pick-up basis; Retrieve and FAX records for release of information according to request for medical emergencies and verifies the identity of the requestor; Validates appropriateness of authorization with records to be released; Copies pertinent parts of patient records for transfer to other health care facilities.
ESSENTIAL FUNCTIONS FOR INPATIENT DISCHARGE AREA: Makes daily rounds to! floors for pickup of medical records on discharged inpatients; Ensures receipt of all medical records on discharged patients at the time of chart pick-up; Sorts records by discharge date and checks off discharge list; Notifies supervisor of discrepancies in admission and discharge dates; Routes loose documents and returned old medical records from floors; Attaches late arriving ancillary department test results and transcribed reports on appropriated chart; Assembles medical record in appropriate order according to area procedures; Analyzes the medical records after discharge to ensure that required reports and signatures are included in the record as specified by standards of regulatory agencies, medical staff by-laws/rules and regulations, department policy and area procedures; Notes deficiencies on patient medical records to be completed by physicians and professional staff. Enters deficiencies according to current incomplete record processing system; Maintains appropri! ate death registries and computes statistics; Prepares and maintains te! mporary patient folders for records in incomplete file.
ESSENTIAL FUNCTIONS FOR OUTPATIENT DISCHARGE AREA : Makes daily rounds to designated outpatient areas for chart pickup; Sorts outpatient records by visit date and files alphabetically; Attaches late arriving ancillary department test results and transcribed reports to appropriate patient medical record; Analyzes the medical records after discharge to ensure that required reports and signatures are included in the record as specified by standards of regulatory agencies, medical staff by-laws/rules and regulations, department policy and area procedures; Notes deficiencies on patient medical records to be completed by physicians and professional staff. Enters deficiencies according to current incomplete record processing system; Maintains appropriate death registries and computer statistics; Prepares and maintains temporary patient folders for records in incomplete file.
ESSENTIAL JOB FUNCTIONS! FOR TRANSCRIPTION AREA: Verifies surgical schedule daily; Verifies the pre-admission book daily for next day surgeries; Calls physicians offices as needed to follow up on transcribed reports; Files copies of dictated reports in physician files daily; Weekly completes appropriate mailing distribution of physician copies; Determines priority dictation and distributes to appropriate transcriptionists; Separates reports from printers and makes rounds to appropriate locations in the hospital to put transcribed reports on patient charts; Verifies whether or not certain medical reports have been dictated and/or transcribed for incomplete record and reprints reports as necessary.
ESSENTIAL FUNCTIONS FOR THE RECEPTION AREA Determines priority and retrieves incomplete records for physicians to complete signatures and dictation; Updates the chart completion system; Maintains and enforces the physician suspension procedure in accordance with Medical Staff Bylaws, rules an! d regulations and hospital procedure; Files and maintains the accuracy ! of the incomplete records in the incomplete record file; Determines, analyzes, interprets and assists others in the completion of the medical records by individual physicians or professional staff; Prepares, routes and communicates updates to the physician delinquent and suspension list; Prepares and distributes incomplete record notices, restriction and suspension letters; Updates chart completion and chart tracking on patient records as appropriate; Routes completed medical records for permanent filing; Performs incomplete chart audits according to area schedule.
ESSENTIAL FUNCTIONS FOR BIRTH CERTIFICATE AREA Obtains necessary information from appropriate personnel, patient and physician to enter information on the state birth certification system and transmit to the Department of Vital Records; Maintains birth log and log of registration fees/necessary forms; Updates computer and patient record of name changes; Routes birth certificate worksheet to newborn recor! d.
ESSENTIAL FUNCTIONS FOR ELECTRONIC DATA SPECIALIST AREA: PREPPER : - This position is responsible for daily records receipt, verification, alphabetizing, and prepping medical records according to specified procedures/guidelines for scanning into the electronic record system for permanent retention. SCANNER : - This position requires ability to operate document scanners efficiently and recognize system errors and make corrections for document entry into the electronic record system, identify scanner problems and seek immediate resolution. QUALITY CONTROL/VALIDATION : - This position conducts quality checks and validates all scanned documents prior to release to the electronic medical record repository. Responsible for correcting all misfed or poorly scanned documents or those scanned in error to the wrong encounter.
ESSENTIAL FUNCTIONS FOR CONCURRENT CHART MANAGEMENT ON FLOORS : Copy admission reports; highlight new admissions for specific floors.! Print list of dictated operative reports from previous and current day! from Voice Writer system. Look up CPI numbers to identify patient names. Look up patient names of newly admitted patients in Med Rite SX system to see if H&P has been dictated. Go to floors and distribute loose material, which includes but is not limited to radiology reports, laboratory reports, MAR sheets, stop order reports and dictated materials. Check charts for any other type of reports that can be used for H&Ps such as consultation reports, current H&Ps from other facilities. Do transcription rounds. Help in Reception Area when time allows.

Knowledge of medical terminology a! nd human anatomy & physiology preferred. Knowledge of medical record documentation requirements as mandated by state and federal regulations and TJC standards preferred Knowledge and understanding of HIPAA and state requirements regarding the confidentiality of patient information Excellent customer relations skills which include the ability to diplomatically handle irate customers in a courteous and professional manner. Excellent verbal/written communication and interpersonal skills are required. Must be assertive and possess the ability to multi task and prioritize tasks. Must be able to perform in a stressful and fast paced environment. Computer knowledge of MS Office. Ability to write reports with management review. Ability to use photocopier machine, facsimile machine, personal computer, adding machine, telephone, paging system, postage machine/scale. Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Abili! ty to deal with problems involving several concrete variables in standa! rdized situations.
EDUCATION / EXPERIENCE

High School Diploma or GED and Medical Terminology and Anatomy Physiology, minimum
Associates degree in Health Information Technology or a related field, preferred
Minimum of 2 years experience in an acute care hospital Health Information Management Department preferred
